Intake Sentry
Send or upload a hospital referral packet. Intake Sentry returns an interactive scorecard in minutes: what qualifies, how strong the fit is, and the evidence behind every flag.
A packet is sent or uploaded and you receive a scorecard back in minutes. Every decision traced to the page it came from.
How it works
STEP 01
Submit a hospital referral packet, faxed pages or a digital export. Intake Sentry reads the whole packet, not a summary.
STEP 02
Before any score, Intake Sentry checks the gates that decide eligibility: an inpatient-stay requirement and a payer-contract match. A missing document is flagged as a gap to resolve, never a guess.
STEP 03
Cleared referrals get a 0 to 100 fit score across five pillars. Every flag cites its source, so reviewers jump straight to the page instead of hunting for it.
The fit score
Cleared referrals are scored 0 to 100, where higher is a stronger fit. The score is built on the same five pillars for every referral, so results are comparable and easy to explain.
Behavioral, patient history and mental health factors.
Clinical, medical acuity and treatment requirements.
Functional, mobility and daily-living support needs.
Financial, payer sources and cost modeling.
Legal, compliance and documentation risks.

Illustrative only. The numbers above are a design sample, not output from a real patient or a real scorecard.
Built for the approval
Referral admission processes vary by facility. Whether it is one admin or a team, Intake Sentry puts every reviewer on the same scorecard, looking at the same evidence. No forwarded faxes, no version confusion.
Social worker
Sees the qualification gates, demographics, and payer match, the questions that decide whether the referral fits at all.
Director of nursing
Reviews the clinical flags against the source pages, confirming what the packet actually supports.
Administrator
Reads the summary and records the decision, with the full scorecard and its evidence behind it.
Privacy by design
Referral packets are de-identified as they come in, and identifiable data is not retained beyond the delivery window. Processing runs inside a Business Associate Agreement-covered AWS environment, designed for HIPAA-covered workflows.
Intake Sentry
Bring a de-identified sample. We will walk you through the scorecard your team would see.
